Application

To participate in the Idaho Dental Education Program you must:

  1. Complete your AADSAS Application indicating:

    a) Idaho as your state of residency,

    b) Creighton as one of the schools you are applying for admission.

  2. Have a copy of your Dental Aptitude Test (DAT) scores sent to Creighton
  3. Complete the Creighton Supplemental Application with the appropriate fee.
  4. Have official transcripts from college course work submitted to Creighton as needed.
  5. Have your personal references or pre-dental committee evaluation sent to Creighton.
  6. Complete the Idaho Residency Certification form and email to Dan Woerner at woerdan@isu.edu.

Once complete, your application will be considered by the Admissions Committee. Due to the competitive nature of the application process, it is recommended that your application is completed by August or early September of the year prior to the time you are seeking enrollment. The admissions committee will generally interview between 20 and 30 applicants in September and October, with accepted students being notified in December. In recent years, all 9 positions in the program are filled by the middle of December of the year prior to starting the program. Applications that are complete after this time will be considered only on a space available basis.

The applicant must be a legal resident of Idaho with standards set by the Idaho Legislature.[WU1]

The completed form for residency certification must be returned by email to Dan Woerner at woerdan@isu.edu before your application can be considered complete. In the event certification of residency is denied, the decision may be appealed through the office of the Idaho State Board of Education. If you have any questions regarding the residency determination worksheet, please reach out to:

All applicants above the program's minimum cut-off score (75%) are invited to participate in an interview. The interview team asks questions from a bank of standardized questions and provides a score to the admissions committee. As part of the IDEP application procedure, applicants may be requested to participate in a personal interview with members of the IDEP Admissions Committee. These interviews are by invitation and are conducted at Idaho State University in Pocatello. Applicants who are unable to complete a personal interview in Pocatello can complete a telephone interview. There is no penalty in the admission process for applicants who complete phone interviews in deference to interviewing in person.
